  P lease read the following tips to make your ordering easy and accurate:

1. Print or type. Be sure to give a complete and accurate street address (no box # please, unless that is all you have)

2. Enclose check or money order made payable to Charlie Makes 'U' Nuts.

3. If you provide a business card of other personal card for gift orders, we will include them with your gift delivery. Please send complete instructions with your order.

4. Specify a delivery date. Allow 1-2 weeks for delivery. Order early for holiday deliveries. If delivery date is not specified, we will assume you want immediate delivery.

5. Shipping charges must be added for each different address.

6. For shipping charges other than regular UPS Ground shipment or U.S. Priority Mail, such as First or Second Day Air delivery, etc., call for extra cost of charges.

7. If additional order forms are needed, please make extra copies as needed.
